What is Ectopic pregnancy?

Ectopic pregnancy occurs when a fertilized egg implants and begins to develop outside the uterus, most commonly in the fallopian tube. This abnormal implantation prevents the embryo from growing normally and can lead to serious complications if left untreated. Symptoms often mimic those of a normal pregnancy initially, including missed periods and breast tenderness, but may progress to include pelvic pain, vaginal bleeding, and shoulder pain. In an ectopic pregnancy, conception occurs outside the uterus when the embryo implants itself elsewhere. Typically, during natural conception, sperm fertilizes the egg within the fallopian tube. The fertilized embryo then travels through the tube towards the uterine cavity, aided by smooth muscle contractions. However, if there’s a blockage or damage to the tube, the embryo’s journey is obstructed. 

This can lead to implantation within the tube, resulting in a tubal pregnancy. Although most ectopic pregnancies occur in the fallopian tube, implantation can also happen in abdominal areas, the ovaries, or the cervix. Early detection and intervention are crucial for managing this condition.

  • These pregnancies may stem from tubal conditions, often linked to past pelvic infections like Chlamydia, Gonorrhea, or other STIs.

How is Ectopic Pregnancy Diagnosed?

The diagnosis of this type of pregnancy involves:

  • Reviewing the patient’s medical history, particularly focusing on the date of her last menstrual cycle.
  • Assessing the levels of the pregnancy hormone, hCG, through a blood test.
  • Conducting an ultrasound examination of the uterine cavity.
  • Performing a pelvic examination to identify any signs of pain, tenderness, or abdominal masses.

Ectopic pregnancies typically display an abnormal pattern of hCG increase, indicating suboptimal development. Monitoring hCG levels can help determine the stage of pregnancy. If the blood test reveals a specific hCG value but the ultrasound fails to detect a pregnancy within the uterine cavity, ectopic pregnancy is suspected. Irregular pregnancies, including ectopic pregnancies and those at risk of miscarriage, may show reduced levels of progesterone. 

In some cases, confirming an ectopic pregnancy requires laparoscopy, a surgical procedure that provides visual evaluation of the abdomen.

Managed

How can ectopic pregnancy be managed?

  • Observation: Careful monitoring of the patient’s condition is essential, especially in cases where immediate treatment is not feasible. This approach is typically recommended for women with early ectopic pregnancies who show minimal symptoms and have low serum hCG levels (below 1,000 IU/L) without intervention.  Regular blood tests are conducted to track the decline in hCG levels, indicating the spontaneous termination of the ectopic pregnancy.
  • Medication: Medical intervention may be appropriate when hCG levels drop below 1500 IU/L and ultrasonographic findings show an adnexal mass with a diameter less than 3.5-4 cm. Methotrexate, a medication aimed at halting the progression of ectopic pregnancy, can be administered. Following the administration of this medication, the patient is closely monitored for an hour.  Possible side effects of Methotrexate include nausea, vomiting, diarrhea, mouth sores, abdominal discomfort, or drowsiness. A follow-up examination of the patient’s hCG levels is conducted after 5 days. If the patient experiences severe abdominal pain during this period, immediate medical attention is necessary.
  • Surgical Intervention: Laparoscopy is commonly employed, particularly if an ectopic pregnancy is diagnosed early before the fallopian tube ruptures. A Laparoscopic Salpingostomy may be performed in such cases, wherein the fallopian tube is accessed through minimally invasive surgery. 

The pregnancy tissue is removed, and the tube is preserved in its original position.

Additionally, their chances of conceiving spontaneously might decrease. Women face a 10 to 15% likelihood of experiencing another ectopic pregnancy following their initial occurrence.
